Our Physician Assistant Dave Forbis, RN, PA-C has been with Urology Associates of NE Florida for over 3 and a half years. Mr. Forbis has specialties in General Urology and Family Practice. He is a former navy corpsman and EMT, has been a registered nurse since 1981, and a physician assistant since 1999. Mr. Forbis is a graduate of the University of North Dakota Physician Assistant Program at Grand Forks and was a former administrative nursing supervisor. Dave is an active speaker in his community for local man to man prostate cancer support groups and speaks monthly at erectile dysfunction seminars. Mr. Forbis is a fellow in the American Academy of Physician Assistants and is listed with International Who’s Who of professional management 1998. He maintains PA certification through the National Commission on Certification of Physician Assistants. |
Our Nurse Practitioner Mary K. Schupp, MSN, ARNP, FNP-C, received her first degree and credentialing as a Registered Nurse in 1979. After 20 years of clinical practice in all areas, including education and administration, and raising five children, she returned to the pursuit of higher education in 1999.
She received her Bachelor of Science in Nursing degree from the University of Phoenix in 2002, and graduated from the University of Southern Indiana with her Master of Science in Nursing degree as a Family Nurse Practitioner in 2005. Mary has completed the only urodynamics training program recognized by the International Continence Society, and is an active member of the Society of Urologic Nurses and Associates, the Sigma Theta Tau International Honor Society of Nursing, and the American Academy of Nurse Practitioners, through which she maintains her national certification as a Family Nurse Practitioner. In her spare time, Mary enjoys spending time with her family, scuba diving and sailing, and serving as a Eucharistic Minister at the Cathedral Basilica in Saint Augustine. |
